- 2nd Most Preferred Course:
- 3rd Most Preferred Institution: (Poly/COE)
- 3rd Most Preferred Course:
- 4th Most Preferred Institution: (COE/IEI)
- 4th Most Preferred Course:
- Your Four UTME Subjects combination (Use of English compulsory)
- Your O’level result Type (WAEC, NECO, NABTEB, GCE etc) and their grades if you’re not an awaiting candidate.

You should not need to go with passport. However, you’ll be snapped while registering in CBT centre in white background.
